Coloring Contest Entry Form

3J Motorsports Coloring Contest Entry Form

You can obtain a coloring page by stopping by the 3J pit area on race night, or by printing your own from the 3J Motorsports website at www.3jmotorsports.com . When the entrant has completed their work, simply follow one of the 3 options below to submit your entry. No matter which method is used to submit your entry, please place the artwork and entry form in a sealed envelope.

Option 1: (Preferred method) Send artwork and completed entry form to:

Option 2: Drop off entry at JD Automotive, 417 S. Kansas Street Green Springs, OH between the

hours of 8AM and 5PM Mon. thru Fri. thru Sept 18^th.

Option 3: Drop off at the 3J trailer in the pit area at any race the 3J attends up until the end of the

9-19-09 race at Fremont Speedway.

The entrants Name & Age must be written on the BACK of the actual coloring page. (Entrants age as of 8-27-09, contest start date)

Please advise if you do not want us to use your child’s last name or photo on the 3J website when we post the contest results.

None of your information will be shared with anybody. The sole purpose for the information requested is to notify prize winners and arrange for prize distribution. All personal information will be destroyed at the contests conclusion.
